Eylani Community Health Network, Inc. (ECHN) is a health and wellness concierge. We specialize in connecting communities to resources needed to achieve the highest quality of life. Our focus is to reduce illness and disease in underserved communities by helping individuals navigate and gain access to spiritual, mental, financial, and physical health service providers.

Background

ECHN was founded in 2015 as a health-focused organization on Chicago’s westside. Its primary objective is to promote wellness in underserved communities. The lack of access to health and wellness resources in “low-income” and underserved communities have led to neighborhoods devastated with poor mental and physical health. Of these impacted communities, those of predominately Black and Latinx backgrounds have the highest incidents of death, multiple health conditions, obesity, mental health disorders, and more. These issues contribute to greater problems such as homelessness, hopelessness, political disenfranchisement, the destruction of the family structure and, in some areas, higher rates of violence. ECHN’s objective is to disrupt this cycle of poor health and poor quality communities by bridging connections to wellness services, health promoting events and family building activities. ECHN’s goal is to reduce health disparities by bringing social and health resources to those who need them most. This organization operates under four core platforms- The Ruth Project-A Mother’s Keeper, Fight Three for Me, A Firm Foundation, and Homes for Health. Using these four platforms ECHN is supporting vulnerable populations and educating families about the importance of being physically, mentally, spiritually and financially strong! Eylani Community Health Network strongly believes in its mission, “Better Health, Better Communities!”

The Ruth Project – A Mother’s Keeper

The Ruth Project promotes all aspects of women’s health and empowerment. It supports mothers and children by pulling together necessary resources and educational tools to help them flourish. The goals are to combat high infant and maternal mortality rate, reduce the number of women and children living in poverty, to increase resiliency after traumatic life events, promote health and ultimately empower women to become leaders in the community. 

Fight Three for Me

Fight Three for Me is a Campaign against the three chronic comorbidities plaguing minority and low-income communities; Hypertension (High Blood Pressure), Diabetes Mellitus I & II, and Hyperlipidemia (High Cholesterol). The goal of this campaign is to raise awareness of the damaging effects of these conditions while empowering community members to reduce their exposure. Additionally, this platform encourages healthy lifestyles through coaching, community accountability, group and individual fitness, healthy eating, and continuous health education.

A Firm Foundation

A Firm Foundation’s purpose is to breed new leaders in the home, neighborhood, and community through family building exercises. It focuses on financial literacy, spiritual wellbeing, personal fitness, and civic engagement as tools for strengthening the family unit. The goal is to expose families to resources for success then send them back out into the community as a catalyst of change. As confident leaders, they can uplift other families to build legacies and become more socially and politically involved.

Homes for Health

Homes for Health is a long-term, development plan that seeks to identify, acquire, and repurpose unused lots and abandoned properties. The goal is to increase wellness of low-income communities by resurrecting these spaces for health-related services and programs. Properties will be used to host seminars, pop-up clinics, health fairs, rental space for healthcare providers, temporary shelters, home births and more.
